They were being overly cautious, I was truly unprepared for the follow-up appointment. My daughter and I were yakking a mile a minute about going shopping afterwards when my cardiologist asked if I wanted the good news or the bad news first. I chose “bad news,” because how bad could it be?

They’d found an aneurysm in the upper aorta of my heart, one that rated 4.9 on a 5-point scale for severity. Translation? The sucker was ready to blow. Just lifting a suitcase could cause a rupture that would kill me. A similar condition claimed the life of actor John Ritter.

It was not hopeless. They could try to repair the aneurysm in a complicated open heart surgery.

In shock, I realized I didn’t know beans about heart health and wasn’t even sure how the heck to spell “aneurysm.” So in the days before my surgery, I armed myself with knowledge. I discovered heart disease is the No. 1 killer of U.S. women, claiming one in four lives.

Included in the options for birth providers are Obstetrician/Gynecologists (OBGYNs), certified nurse-midwives (CNMs), or certified professional midwives (CPMs). There are also other providers not described in this article including Family Physicians, Certified Midwives, and lay midwives. Certified Nurse-Midwives also practice mostly in the hospital, although in many cities and states will also practice in home and birth center settings. They have nursing licenses and master’s degrees in nurse midwifery. They are certified by the American College of Nurse Midwifery through a board exam. Although midwives do specialize in low risk pregnancy, they do manage high risk pregnancies through co-management with obstetricians. It is an option to use pain medication with nurse-midwives, however they will do labor support for natural childbirth. Nurse-midwives also assist in cesarean sections.

Certified professional midwives are educated through a specialized midwifery school, self study, and/or apprenticeship. Their training meets the standards of the North American Registry of Midwifery. They only deliver in home or birth center settings. Because of their training and because of where they deliver they can only manage low risk pregnancies. They always will support women in labor with natural childbirth and but offer any pain medication or epidurals in labor. If an emergency happens they will need to transfer the patient to the hospital for a cesarean section with an obstetrician.

These types of providers are the three most popular options women choose from for care during their pregnancy. Based on what preference a woman has for birth setting, pain management, and pregnancy philosophy, the decision of which provider to use is very personal. Most important is that the woman is comfortable with the setting and finds a provider she can trust. It is worthwhile to look into the laws in the state where she is living as some states have very different regulations when it comes to home birth.
